Literal translation: The name, please tell.
Free translation: Please tell me your name.
Formal
お名前を、教えてくれませんか?
(onamaê wo, oshiêtê kurêmasênka?)
Literal translation: The name, can you tell?
Free translation: Do you mind telling me your name?
