Depends on the context. If it’s just ¿Qué tal? or if it’s following an “¡hola!” – then that’s just a greeting, informal, equivalent to “What’s up?” or “How’s it going?”
It can also be used to ask how something is.
¿Qué tal está tu comida? – How’s your meal?
Or as “What if..”
¿Qué tal que quiera lastimarme? – What if he wants to hurt me?
Or when giving an option “How about..”
¿Qué tal un poco de esto. – How about a little of this.
